Connecting Communities is a registered NDIS provider, delivering supports since 2017 to the Townsville community and surrounding areas. Our focus is on those individuals with a disability to provide meaningful supports in assisting clients to achieve their goals.
Services Provided:
Community Access-We provide the necessary supports to enable people to become a confident and contributing member of the community driven by the goals and needs of the individual.
Assisted Daily Living-Our dedicated Support Workers can assist you with your daily personal care and activities.
Nursing-Our Nursing team are able to provide initial assessments, care plans, training for both carers and clients, monitoring of care plans, and ongoing visits from nurses.
Accommodation-SDA, Respite, and Short Term accommodation.
