Connections for Community is a Townsville-based nurse-led team, dedicated to providing personalised in-home nursing support and practical care navigation for individuals and families who are recovering at home, managing illness, or feeling unsure how to access services. We work in partnership with GPs and other care providers to ensure continuity of care, helping people feel heard and reducing overwhelm. We travel across regional and rural areas, ensuring communities outside major centres receive support.
Gift a potted plant for a loved one coming home from hospital, on birthdays, special occasions or a bereavement date.
We deliver the potted plant, the visit provides an informal nursing session to build rapport, discuss needs and create gentle linkages to services. The point is to give the gift of connection.
