Lifestyle Connections Association Inc. (formally known as Leisure Connections Association Incorporated) is a family governed not for profit organisation. We assist people with a disability in the Cairns Area to plan what support they need to live an ordinary life within their community. We employ support people to assist individuals on a one-to-one basis to participate in their chosen recreation, leisure and lifestyle activities in generic community settings.

NDIS SERVICESHOW WE ASSIST
Participate in CommunityAssist our Service User to participate actively in community, civic, social, recreational and leisure activities; includes supporting our Service User during these activities, and developing their ability to partake in these activities. Lifestyle Connections has always and will continue to provide support on a one-on-one basis in all areas of home and community life where the Service User has choice and control and is fully included. Our Service User and his/her support worker are seen as two people participating together in our community just like everyone else, so we set by example.
Assist with Personal ActivitiesAssist with and/or supervise personal tasks of daily life in the home and community to develop skills of our Service User and enable them to live as independently as possible. These individual supports can be provided in a range of environments, including but not limited to, the Service User’s own home, and include sleepover support.
Assist with Personal Activities – HighAssist with, and/or supervise, personal tasks of daily life to develop skills of our Service User’s with higher or complex needs, to enable them to live as autonomously as possible. These supports are provided by a worker with additional qualifications and experience relevant to the Service User’s needs. These individual participation supports can be provided in a range of environments, including the Service User’s own home, and social and community participation.
Support Co-ordinationProvide support co-ordination for Service User’s with more complex needs or high-level risks and a more complex NDIS Plan. Support is time limited and focuses on addressing barriers and reducing complexity in the support environment, while assisting the Service User to connect with supports and build capacity and resilience. It may also involve development of an intervention plan which will be put in place by disability support workers.
Life Skills DevelopmentAssist our Service User to develop their daily living skills to increase independence, and their social skills for improved participation in community and social activities. This capacity building support focuses on training and development activities undertaken by the Service User and/or their carer to increase the Service User’s ability to live as autonomously as possible. Includes supports that will enhance the ability of the Service User to travel and use public transport independently and to manage their Plan.
Household TasksAssist our Service User to maintain their home by performing essential household tasks. This may involve undertaking essential household tasks that the participant is not able to undertake (meal preparation and delivery, house or yard maintenance, cleaning and linen).
Accommodation/TenancySupport to guide, prompt, or undertake activities to ensure our Service User obtains/retains appropriate accommodation. May include assisting to apply for a rental tenancy or undertake tenancy obligations. Includes transition supports only.
Assist Life Stage, TransitionAssist during life-stages and transition periods in coordinating and managing daily living activities and community access. May include support connection, co-ordination of supports, assistance with accommodation and tenancy obligations, life transition planning and general assistance with decision making, daily planning and budgeting. Includes short and long-term supports that focus on strengthening the Service User’s ability to coordinate their supports, and to assist them to live at home and participate in their community. May include help establishing volunteer assistance within their home or community to develop skills e.g. assistance in attending appointments, shopping, bill paying, taking part in social activities and maintaining contact with others.
Plan ManagementLifestyle Connections can act as the financial manager for your NDIS Plan. This includes making payments to Providers, expense claims processing, providing monthly statements and claiming payment from the NDIA. As a Plan Manager we also liaise with providers and Service Users to implement and manage the plan. We can be a Financial Intermediary as we deliver plan management supports and possess good knowledge of providers and supports, developed through strong connections in the relevant community. Therefore, we can help strengthen the Service User’s ability to undertake tasks associated with the management of their supports, including building financial skills, organisational skills, and enhancing their ability to direct their supports and/or develop self-management capabilities.
Daily Tasks/Shared LivingAssist with tasks of daily life for our Service User in a shared living environment, which is either temporary or ongoing, with a focus on developing the skills of each individual to live as autonomously as possible. Includes short-term accommodation and assistance (e.g. respite care), different from usual arrangements – may include short stays in group-based facility, or purchase of additional in-home support. Includes all expenses in 24 hr period incl. assistance with daily personal activities, accommodation, food & negotiated activities. Is for short periods of up to 14 days at a time.
